TWO STEPS TO JOIN SFUPS:
STEP 1:
Complete and sign the Membership Questionnaire Form.
(A separate form must be completed for each member in a Family Membership.)
STEP 2:
Complete the short Membership Application below, choose your membership level via the dropdown box and pay dues securely via credit card.
(Each member in a Family Membership must fill in the application below. The primary member will select the “Family Membership” payment option;
the additional members will select “Additional Family Member”, which has no fee. Each additional member must have a unique Email ID in order to obtain and manage a separate account)
Your membership will be considered final upon receipt of your payment and completed Membership Questionnaire. Thank You!
NOTE: If you do not want to pay by credit card, Membership Dues can be paid via a check. You will still need to complete the Membership Application below and select the “Pay by Check” option. Checks should be made out to SFUPS. Email membership@sfups.org and you will be advised where to mail the check.
Annual dues are $50.00 for individuals and $65.00 for families.
If joining after July 1st, the membership dues are pro-rated by 50% for new members only.
If joining after October 1st, the membership dues are pro-rated by 75% for the current year.
If joining after November 1st, new members pay only the dues for the following year.
